III. Application Requirements£º
1. The "Shanghai Housing Provident Fund Account (Supplementary Housing Provident Fund) Application Form for Suspension of Payments (Form B) for Supplementary Housing Provident Funds)" completed and affixed with the official seal of the unit or the special seal for housing provident fund business;
2.Proof materials that meet the conditions of account cancellation;
3. If the applicant unit owes the payment, the unit and individual should confirm the payment and deposit status. In addition to the information corresponding to the above situation, the unit should also provide the employee labor contract, labor manual, salary base certificate, unit payment list and other supporting materials.
IV. Handling process:
The unit provides the necessary materials-the unit's housing provident fund account is located at the CCB housing provident fund business branch to review and handle it.
V. The processing time limit:
When the procedures are complete, no more than 3 working days.
